Human Review for AI Actions
Human review is the accountable decision step in which an authorized person examines material evidence, uncertainty, affected people, and the proposed path before a consequential action proceeds.
When review matters most
Review is especially important where uncertainty is material, consequences are difficult to reverse, affected people have limited power, or the request conflicts with a constitutional safeguard. ESCALATE is a signal that the record needs accountable attention, not a handoff that removes responsibility.
What a reviewer should be able to see
A meaningful review should expose the request, assumptions, source context, uncertainty, proposed outcome, alternatives, and reason. An override should be attributable rather than silently replacing the evaluation.
For example, a clinical triage assistant may organize a question, but a qualified clinician must retain the authority to assess evidence and decide what care is appropriate.
